Dental Services
Treatment Consent
By scheduling an appointment and receiving treatment, you consent to the dental care recommended by Dr. Shannon Skiba and our dental team. We will always explain procedures and obtain specific consent before treatment.
Appointment Policy
- Please arrive on time for your scheduled appointments
- We require 24-hour notice for cancellations or rescheduling
- Repeated no-shows may result in a fee or inability to schedule future appointments
- Emergency appointments are available for urgent dental needs
Financial Responsibility

- Many dental insurance plans (we will file claims on your behalf as a courtesy)

Insurance
While we will submit insurance claims as a courtesy, you are ultimately responsible for all charges not covered by your insurance plan. Insurance benefits are an agreement between you and your insurance company.

- How to opt in: by submitting our online text message sign-up form, by providing and confirming your mobile number with our office, or by texting us first.
- Message frequency varies based on your appointments and account activity.
- Message and data rates may apply according to your mobile carrier and plan.
- To opt out, reply STOP to any message at any time. For help, reply HELP or call (770) 478-1001.
- Consent is not a condition of receiving dental care or any other service.
- Mobile carriers are not liable for delayed or undelivered messages.
- Your mobile information is never sold or shared with third parties or affiliates for marketing purposes. Text messaging opt-in data and consent are not shared with any third parties.
Governing Law
These Terms of Service are governed by the laws of the State of Georgia. Any disputes shall be resolved in the courts of Clayton County, Georgia.
Changes to Terms
We reserve the right to modify these terms at any time. Changes will be posted on our website and in our office. Continued use of our services after changes constitutes acceptance of the new terms.
